Propranolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for Propranolol: your body relies partly on an enzyme called CYP2D6 to break this beta blocker down. In lab testing, plumbagin (a compound in Ceylon Leadwort) slowed that same enzyme. If that played out in real life, propranolol could build up a bit more than usual, nudging its effects stronger. That might show up as a heart rate that runs too slow, lower blood pressure, lightheadedness, or feeling unusually tired.

The key thing to know is that this is only test-tube evidence, and it has never actually been reported in people, so the real-world risk looks low. Still, I'd run it past your pharmacist or doctor before combining them, especially if you already notice dizziness or a slow pulse.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the short version: your body uses an enzyme called CYP1A2 to help clear propranolol. A compound in Ceylon Leadwort, plumbagin, has slowed down that same enzyme in lab dishes. If that were to happen in people, propranolol could build up a bit more than usual, which might lead to stronger effects like a slower heart rate, lower blood pressure, lightheadedness, or feeling unusually tired.

The important caveat: this is only seen in test-tube and animal research, not in actual patients, so it's theoretical at this point. I wouldn't lose sleep over it, but I also wouldn't ignore it. Before combining the two, please run it past your pharmacist or doctor so they can keep an eye on your blood pressure and pulse.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Propranolol (Cardiovascular) Used For?

Brand names: Inderal LA, Inderal, Angilol, Syprol, Inderal Injection, Half-Inderal LA, Bedranol SR, Half Beta Prograne, Beta Prograne, Lopranol LA, Slo-Pro, InnoPran XL

This intravenous (IV) form of propranolol is used in hospital or clinical settings to treat dangerous heart rhythm problems. It is generally reserved for life-threatening arrhythmias (abnormal heart rhythms) or those that occur during anesthesia.

Specific conditions it treats include fast heart rates coming from the upper chambers of the heart (supraventricular tachycardia), certain arrhythmias caused by digitalis toxicity, dangerous ventricular rhythms that haven't responded to other treatments, and fast heart rates caused by excess adrenaline-like substances during surgery.
