Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body relies on a liver enzyme called CYP2D6 to clear dextromethorphan, the active ingredient in many cough syrups. A compound in Ceylon Leadwort, called plumbagin, has been shown to slow down that same enzyme, but only in lab dish studies so far. The theory is that if CYP2D6 is blocked, dextromethorphan could build up in your system more than expected.
If that happened, you might feel more of its side effects, like dizziness, drowsiness, confusion, or an upset stomach. I want to be honest though: this has never been reported in real people, so it's a possibility on paper, not a proven problem. Still, if you plan to use both, it's worth a quick chat with your pharmacist or doctor first.

Here's the gist with this pairing. Ceylon Leadwort contains plumbagin, which in test-tube studies appears to slow down a liver enzyme called CYP3A4. Dextromethorphan is partly broken down by that same enzyme, so in theory the supplement could let more of the medicine linger in your body. If that happened, you might notice stronger side effects like drowsiness, dizziness, or feeling a bit foggy or jittery.
I want to be honest about the strength of this, though: the concern comes only from lab work, not from real people taking the two together. So it's more of a 'keep an eye out' than a red alert. Still, if you plan to use Ceylon Leadwort regularly, run it past your pharmacist or doctor first so they can look at your full list.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.
Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
