Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the situation with Ceylon Leadwort and your seizure medicine, perampanel (Fycompa). Your body partly relies on a liver enzyme called CYP1A2 to break perampanel down. A compound in this plant, plumbagin, has been shown to slow that enzyme down, but only in lab and test-tube studies so far. The thinking is that if it slows clearance, perampanel could build up a bit more than usual.

If levels did climb, you might notice more of perampanel's typical effects, like dizziness, drowsiness, trouble walking, or mood and irritability changes. That said, this is theoretical and hasn't been reported in real people. I wouldn't be alarmed, but since perampanel needs steady dosing, please check with your pharmacist or doctor before adding this supplement.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the basic idea with Perampanel and Ceylon Leadwort. Your seizure medicine is partly broken down by a liver enzyme called CYP2C9. A compound in this plant, plumbagin, has been shown to slow that enzyme down in lab testing. If that happens in the body, Perampanel could clear a bit more slowly and build up higher than expected.

With more Perampanel hanging around, you might notice stronger side effects like dizziness, sleepiness, trouble with balance, or mood changes (Perampanel already carries a warning about irritability and behavior). I want to be honest though, this interaction has only been seen in the lab, not in real people, so it's theoretical. Still, since you don't want surprises with a seizure drug, please check with your pharmacist or doctor before combining them.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
