Diclofenac Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist for diclofenac (Cataflam, Voltaren). Your liver uses an enzyme called CYP2C9 to break this medicine down. In lab studies, a compound in Ceylon Leadwort called plumbagin slowed that same enzyme. If that holds true in the body, diclofenac could clear more slowly and build up a bit, which may nudge its effects and side effects higher. With an NSAID like diclofenac that can mean stomach upset, heartburn, a risk of stomach bleeding, fluid retention, or higher blood pressure.

That said, this is only test-tube evidence and hasn't been seen in real patients, so I wouldn't panic. Still, since diclofenac already carries stomach and heart cautions, it's worth a quick chat with your pharmacist or doctor before pairing the two.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing about diclofenac potassium: it's an NSAID, so it already nudges up your bleeding risk on its own by affecting how your platelets stick together and by irritating the stomach lining. Ceylon Leadwort may do something similar. In animal studies, its root extract reduced platelet clumping and lengthened bleeding time, so stacking the two could, in theory, add up to more bleeding risk, things like easy bruising, nosebleeds, or stomach upset that turns into bleeding.

I want to be straight with you, though: this concern comes from lab and animal work, not from human studies, so it's far from proven. Still, since both can lean the same direction, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you also take aspirin or a blood thinner.
