Why this applies to Tacrolimus

Tacrolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for tacrolimus: your body relies on an enzyme called CYP3A4 to break this medication down and keep its blood levels in a safe window. A compound in Ceylon Leadwort, called plumbagin, has been shown to block that enzyme, at least in lab dish studies. If that same blocking happened in a person, tacrolimus could pile up and reach higher levels than intended.

Why that matters: tacrolimus is a narrow therapeutic index drug, so even small swings can tip you toward side effects like kidney trouble, tremors, headache, high blood pressure, or high potassium. The flip side, levels dropping too low, risks organ rejection. So far this is only theoretical and based on lab data, not real patients. Still, with a transplant on the line, please check with your transplant team or pharmacist before using it.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the situation with tacrolimus and Ceylon Leadwort. Tacrolimus is a transplant medication that works by quieting your immune system so your body doesn't attack the new organ. It's a very precise drug, meaning even small shifts in how it behaves can matter a lot.

The concern here is mostly theoretical. In lab and animal studies, plumbagin (a compound in this plant) showed its own immune-calming effects. So in principle it could add to or muddle the careful balance your transplant team has set with your tacrolimus. There aren't real human studies showing this happens, so I wouldn't panic. But because keeping your tacrolimus level just right is so important, I'd skip this supplement unless your transplant doctor or pharmacist clears it first.

Tacrolimus is prescribed to help prevent your body from rejecting a transplanted organ. It is used in adults who receive a kidney, liver, or heart transplant, and in children who receive a liver transplant. It is always used alongside other anti-rejection medicines.

Because tacrolimus works by dampening your immune system, it lowers the risk of your body attacking the new organ — but it also means you need to be watched closely for infections and other side effects throughout your treatment.
