Why this applies to Apixaban

Apixaban falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the thing to know if you take Eliquis (apixaban): your liver partly clears it using an enzyme called CYP3A4. In lab dishes, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. If that happened in your body, apixaban could clear more slowly and build up, which mainly raises your bleeding risk. Watch for things like easy bruising, nosebleeds, bleeding gums, blood in urine or stool, or cuts that won't stop.

I want to be honest about the evidence, though. This is only test-tube data and has never been reported in real people. Still, since apixaban is a blood thinner where small changes matter, please check with your pharmacist or doctor before combining them.

Apixaban also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the gist for you: Apixaban (Eliquis) is already a blood thinner, and Ceylon Leadwort may have a mild blood-thinning effect of its own. In animal studies, its root extract made platelets stick together less and made bleeding take longer. Stacked on top of Eliquis, that could in theory nudge your bleeding risk up, meaning things like easy bruising, nosebleeds, bleeding gums, or in rare cases more serious bleeding.

That said, this concern comes only from lab and animal research, not from human studies, so it's far from proven. Still, since Eliquis is a drug where bleeding is the main worry, I'd play it safe. Please check with your pharmacist or doctor before adding this supplement rather than starting or stopping anything on your own.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
