Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with this one: hydrocodone is actually a prodrug. Your liver uses an enzyme called CYP2D6 to turn it into hydromorphone, which is the part that gives stronger pain relief. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dishes to slow that same enzyme down. So in theory, less of your hydrocodone would get converted, and you might feel less pain relief than expected, not more.

I want to be honest about how thin this evidence is. It comes only from test-tube studies, not real people, so we genuinely don't know if it matters in everyday life. The pseudoephedrine part isn't really affected here. Before adding this supplement, just run it by your pharmacist or doctor so they can check on your whole list.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist with this combo product. The hydrocodone part is partly handled by a liver enzyme called CYP3A4. A compound in Ceylon Leadwort, plumbagin, slowed that enzyme down in lab dish studies. In theory, if your body clears hydrocodone more slowly, more of it could hang around. That could mean stronger effects like extra drowsiness, dizziness, constipation, or in a worst case slowed breathing. The pseudoephedrine side isn't really cleared this way, so that part is less of a concern here.

One big caveat: this is only test-tube data, never confirmed in people, so I wouldn't panic. Still, since hydrocodone is an opioid, I'd run it by your pharmacist or doctor before pairing the two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.
