Hydrocodone, Pseudoephedrine, Chlorpheniramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with this combo. The hydrocodone in Zutripro is a prodrug that your liver partly converts (using an enzyme called CYP2D6) into hydromorphone, the form that does most of the heavy lifting for pain. Lab studies suggest a compound in Ceylon Leadwort, plumbagin, can block CYP2D6. In theory, that could mean less hydromorphone gets made, so you might feel less pain relief than expected.

I'll be honest: this is based only on test-tube research, not real people, so it may not pan out. The chlorpheniramine and pseudoephedrine pieces aren't the main concern here. Still, before pairing them, please check with your pharmacist or doctor.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

The piece of Zutripro I want to focus on here is the hydrocodone, since your liver uses an enzyme called CYP3A4 to help break it down. In lab studies (not in people yet), plumbagin from Ceylon Leadwort appears to slow that enzyme down. If that held true in the body, hydrocodone could clear a little more slowly and build up, which might mean more drowsiness, dizziness, constipation, or slowed breathing.

I'll be honest: this is only test-tube evidence, so I'm not ringing alarm bells. Still, opioids are nothing to gamble with, especially combined with an antihistamine like chlorpheniramine that already makes you sleepy. Please run this combination by your pharmacist or doctor before pairing them.

Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
