Why this applies to Perphenazine

Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Your body relies on a liver enzyme called CYP2D6 to help clear perphenazine. Plumbagin, the active compound in Ceylon Leadwort, has been shown to slow that same enzyme down, but only in lab dish (test tube) studies. In theory, if this happened in real people, perphenazine could build up a bit higher than expected. That could mean more of its typical side effects, like drowsiness, dizziness, low blood pressure when standing, restlessness, or muscle stiffness and tremor.

I want to be clear that this is a theoretical concern only. It has not been reported in actual patients, so the real-world risk is uncertain. Still, since perphenazine works on the brain, it's worth a quick chat with your pharmacist or doctor before combining the two.

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
