Here's the thing to know about pairing this herb with perphenazine (Trilafon). Rauvolfia vomitoria contains reserpine-like alkaloids that calm the nervous system and lower blood pressure, and perphenazine is an antipsychotic that does some of the same things. Stacking two medicines that pull in the same direction can amplify each other's effects.

In practice, that could mean stronger drowsiness, dizziness, low blood pressure (especially when standing up), and a higher chance of movement-related side effects like stiffness, tremor, or restlessness. There's also a theoretical worry about worsening mood or depression. To be clear, this caution is based on how the compounds work rather than hard human studies, but the potential is serious enough that I wouldn't combine them on your own. Please check with your doctor or pharmacist first.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the basic idea with these two together. Your body relies on a liver enzyme called CYP2D6 to clear perphenazine out of your system. Rauvolfia vomitoria contains a little yohimbine, and in lab studies yohimbine appears to slow that same enzyme down. If that happens in real life, perphenazine could linger longer and build up, which may mean stronger effects and more side effects than usual.

For perphenazine, that could look like extra drowsiness, dizziness, low blood pressure when you stand, dry mouth, or muscle stiffness and restlessness. I'll be honest, this is theoretical and based only on lab data, so we don't know how much it matters in people. Still, rauvolfia also strongly affects blood pressure and mood on its own, so please check with your doctor or pharmacist before combining them.

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
