Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rauvolfia Vomitoria interacts with.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the gist for nortriptyline. Your body relies on a liver enzyme called CYP2D6 to break this antidepressant down and clear it out. Rauvolfia vomitoria contains a small amount of yohimbine, and lab studies suggest yohimbine can slow that enzyme down. If CYP2D6 is working slower, nortriptyline could build up higher than expected in your system.

More nortriptyline can mean stronger effects and more side effects, like dry mouth, constipation, drowsiness, dizziness on standing, or a faster heartbeat. To be fair, this is only a theoretical concern based on test-tube research, not real patient cases, so don't panic. Still, since this plant strongly affects blood pressure and mood on its own, please check with your pharmacist or doctor before combining the two.

TRICYCLIC ANTIDEPRESSANTS (TCAs)

Nortriptyline also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, concomitant use might alter the effects of Rauvolfia vomitoria and increase the risk for adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine, a constituent of Rauvolfia vomitoria, 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. Also, concomitant use of TCAs with Rauvolfia vomitoria may decrease the effects of rauwolfia alkaloids.

Here's the gist if you're thinking about combining Rauvolfia vomitoria with your nortriptyline (Pamelor). This plant is a complex mix of compounds, and the tricky part is they can pull in two different directions. One of its constituents, yohimbine, can stir up the nervous system. In a small study, people already taking a tricyclic antidepressant who got yohimbine ran into severe anxiety, jitteriness, agitation, and tremor. So pairing the two could amplify that on-edge, shaky feeling.

At the same time, the blood-pressure-lowering reserpine in the root may work less well alongside a TCA. To be clear, most of this is theoretical, based on how the chemicals behave rather than hard proof. Nortriptyline isn't a prodrug, so this isn't about activation. Still, I'd run this combo by your pharmacist or doctor before mixing them.
