Lofepramine is a tricyclic antidepressant, and one of its common effects is drowsiness, especially when you're first starting it. Rauvolfia vomitoria contains reserpine and related alkaloids that can also calm the nervous system and lower alertness. So the worry here is simple addition: put two things that make you sleepy together, and you may feel more tired, foggy, or sluggish than you'd expect from either one alone.

I'll be honest with you though, this is a theoretical concern based mostly on animal studies, not strong human data. There's also a separate issue: reserpine can affect mood and the same brain chemicals an antidepressant is trying to support, which is worth flagging. Please don't start or stop anything on your own. Run this combination past your doctor or pharmacist first.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the gist for you. Lofepramine is a tricyclic antidepressant that your liver helps clear using an enzyme called CYP2D6. Rauvolfia vomitoria carries small amounts of yohimbine, and lab studies suggest yohimbine can slow that same enzyme down. In theory, if the enzyme works slower, lofepramine could build up a bit more in your system and its effects (and side effects) could get stronger.

With a tricyclic, that might mean more drowsiness, dry mouth, constipation, dizziness on standing, or a faster heartbeat. I want to be honest though: this is based only on test-tube research, so it's more of a caution than a proven problem. On top of that, this plant has its own strong effects on blood pressure and mood, which is another reason to loop in your doctor or pharmacist before combining the two.

TRICYCLIC ANTIDEPRESSANTS (TCAs)

Lofepramine also falls under TRICYCLIC ANTIDEPRESSANTS (TCAs).

Theoretically, concomitant use might alter the effects of Rauvolfia vomitoria and increase the risk for adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine, a constituent of Rauvolfia vomitoria, 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. Also, concomitant use of TCAs with Rauvolfia vomitoria may decrease the effects of rauwolfia alkaloids.

Lofepramine is a tricyclic antidepressant, and Rauvolfia vomitoria is a potent plant that affects blood pressure and brain chemistry. Here's the concern: this root contains yohimbine-related compounds, and in a small study, people already on a tricyclic who received yohimbine developed severe anxiety, agitation, and tremor. So pairing the two could stir up jittery, on-edge feelings rather than calm.

There's a second wrinkle. Tricyclics like lofepramine can blunt the blood-pressure-lowering action of the reserpine in this plant, so it may not work as intended. Keep in mind this is a theoretical, pharmacology-based caution, not a heavily proven one. Lofepramine is not a prodrug, so the worry is mainly added stimulation. Please check with your pharmacist or doctor before combining them, and don't change anything on your own.
