Here's the concern with pairing Rauvolfia vomitoria and brexpiprazole (Rexulti). Both act on the same brain chemicals. Brexpiprazole adjusts dopamine and serotonin signaling, while rauvolfia's main alkaloid, reserpine, depletes those same messengers and can lower blood pressure. Used together, their effects can stack up. Brexpiprazole is active on its own (not a prodrug), so this isn't about activation, it's about two depressant-type effects piling on.

In real life, that could mean more drowsiness, dizziness or lightheadedness when standing, low blood pressure, and a higher chance of movement-related side effects. This warning is theoretical, based on how the drugs work rather than reported cases, but the potential is serious. Please loop in your doctor or pharmacist before combining them.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the basic idea with these two together. Your body relies on a liver enzyme called CYP2D6 to clear brexpiprazole (Rexulti) out of your system. Rauvolfia vomitoria contains a small amount of yohimbine, and lab studies suggest yohimbine can slow that same enzyme down. If the enzyme works more slowly, brexpiprazole may build up a bit higher than expected, which could mean stronger effects and more side effects like drowsiness, restlessness, dizziness, weight gain, or feeling shaky.

I'll be honest with you though: this is a theoretical concern based on test-tube research, not real patient cases, so the risk may be small. Still, this plant strongly affects blood pressure and mood on its own, so please check with your doctor or pharmacist before combining them.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
