Why this applies to Perphenazine

Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the short version: your body uses an enzyme called CYP2D6 to help clear perphenazine. In lab studies, 1,3-DMAA blocks that same enzyme. If that holds true in people, perphenazine could build up in your system and hit harder than expected. That might mean more drowsiness, dizziness, a dry mouth, low blood pressure, or muscle stiffness and restlessness (the movement side effects this drug is known for).

I'd flag something bigger here, though. DMAA is a banned, unsafe stimulant that raises blood pressure and heart rate, which is a rough mix with an antipsychotic. The enzyme evidence is only test-tube level, so it's theoretical, but DMAA itself is the real concern. Please skip it and check with your doctor or pharmacist.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
