Why this applies to Pirfenidone

Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for Esbriet (pirfenidone): your body relies on a liver enzyme called CYP1A2 to clear this medicine out. A compound in Ceylon Leadwort, plumbagin, has been shown in lab dish studies only to slow that same enzyme down. If that held true in people, pirfenidone could build up a bit more than expected, which might mean stronger side effects like nausea, stomach upset, tiredness, dizziness, or more sun-sensitive skin rashes.

The important caveat: this has never actually been reported in humans, so it stays theoretical for now. Still, because pirfenidone already comes with notable side effects, I'd keep things simple. Chat with your pharmacist or doctor before pairing this supplement with your medication rather than starting it on your own.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
