Why this applies to Rabeprazole

Rabeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the short version: a compound in Ceylon Leadwort called plumbagin can slow down a liver enzyme called CYP2C9 in lab tests. The thinking is that if that enzyme works slower, drugs it breaks down could build up a little. Rabeprazole is partly handled by this pathway, so in theory its levels might edge up, which could mean more of its usual effects like headache, stomach upset, or diarrhea.

That said, I want to keep this in perspective. This is only seen in test-tube and animal research, never reported in real people, and rabeprazole is generally a very forgiving, low-risk medicine. So I wouldn't lose sleep over it, but since this supplement isn't well studied in humans, please run it by your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the short version: rabeprazole (Pariet) is partly cleared by a liver enzyme called CYP3A4. A compound in Ceylon Leadwort, called plumbagin, has been shown to slow that same enzyme down, but only in lab dish studies, not in real people. The idea is that if the enzyme works slower, a bit more rabeprazole could linger in your system.

Honestly, I wouldn't lose sleep over this one. Rabeprazole is a pretty forgiving drug, and even a small bump in levels usually just means mild things like headache, nausea, or some belly upset. Because the evidence is so weak and theoretical, the bigger reason to be careful is plumbagin itself, which can irritate tissues. Still, run it by your pharmacist or doctor before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Rabeprazole Used For?

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
