Mometasone, formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the honest picture with Zenhale (the inhaler that combines mometasone, a steroid, and formoterol, a long-acting bronchodilator) and Ceylon Leadwort. The concern on file is that plumbagin, a compound in this plant, may slow down a liver enzyme called CYP2D6. In theory, that could let certain drugs build up. But this has only been seen in lab dishes, never in actual people, so it's a very soft, theoretical signal.

On top of that, your inhaled medicines act mostly right in the lungs and don't lean heavily on CYP2D6 the way pills like tramadol or fluoxetine do. So I wouldn't expect a real problem here. If you did notice more jitteriness, a faster heartbeat, or tremor from the formoterol, mention it. Always run new supplements past your pharmacist first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the honest picture for your Zenhale inhaler (mometasone plus formoterol). On top of that, mometasone and formoterol aren't mainly handled by CYP2C9 (mometasone leans on a different enzyme, CYP3A4). And since you inhale them, very little reaches your bloodstream to begin with. A theoretical bump might mean slightly more steroid side effects or a faster heartbeat or jitteriness from the formoterol, but that's unlikely here. Still, run any new supplement past your pharmacist first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Your Zenhale inhaler combines mometasone (a steroid) with formoterol (a long-acting bronchodilator). Both rely partly on the liver enzyme CYP3A4 to be cleared from your body. Ceylon Leadwort contains plumbagin, which in lab dishes has been shown to slow down that same enzyme. In theory, that could let more of the inhaled medicine linger, nudging up the chance of side effects like a racing or pounding heartbeat, jitteriness, headache, or, over time, more steroid-related effects.

Here's the honest part: this is based only on test-tube research and has never been reported in real people. So I wouldn't lose sleep over it. Still, since these effects could matter for your heart and breathing, please mention the supplement to your pharmacist or doctor before combining them.
