Why this applies to Pomalidomide

Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist: Pomalidomide is partly cleared by a liver enzyme called CYP1A2. Ceylon Leadwort contains plumbagin, which in lab dish studies has slowed that enzyme down. If the same thing happened in your body, your liver might break Pomalidomide down a little less, letting more of it linger and possibly raising the chance of side effects like low blood counts, fatigue, infections, or blood clots.

I want to be honest about the evidence, though. This has only been seen in test tube and animal research, never confirmed in real people, so it's theoretical at this point. Still, Pomalidomide is a serious cancer drug with real risks, so please don't add Ceylon Leadwort without first checking with your oncologist or pharmacist.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help break down pomalidomide (Pomalyst). Ceylon Leadwort contains plumbagin, and in lab tests plumbagin slowed that same enzyme down. If it does the same thing inside a person, pomalidomide could clear out more slowly and build up to higher levels than expected.

With pomalidomide, higher levels could mean stronger side effects like low blood counts (raising infection and bleeding risk), fatigue, and a higher chance of blood clots. That said, this is only a theoretical concern from test-tube work, not something seen in real patients. Still, because this is a serious cancer medicine, please don't combine them without checking with your oncology team or pharmacist first.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the thing to keep in mind with Pomalyst (pomalidomide): it works partly by adjusting your immune system to fight multiple myeloma or Kaposi's sarcoma. Ceylon Leadwort contains plumbagin, which has shown its own immune-dampening activity, but only in lab and animal studies. So far there's no solid human evidence that the two clash, which is why I'd call this more of a theoretical concern than a proven one.

That said, mixing two things that both tug on the immune system isn't something I'd guess about on your own. Since pomalidomide is a serious cancer therapy with a careful dosing plan, please loop in your oncology team or pharmacist before adding Ceylon Leadwort, rather than starting or stopping anything yourself.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
