Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist: your body uses an enzyme called CYP3A4 to help clear dextromethorphan, the cough suppressant in products like Benylin and Theraflu. Lab studies suggest cannabis can mildly slow that same enzyme. In theory, that could let a bit more dextromethorphan linger in your system, which might bump up its usual side effects like drowsiness, dizziness, or feeling foggy.

I want to be honest with you, though: this is only based on test-tube research, not real people, so the effect is likely small for most folks. Still, if you use cannabis regularly and take dextromethorphan, just keep an eye on how sleepy or off-balance you feel, and check with your pharmacist or doctor before combining them.

What Is Dextromethorphan Used For?

Brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
