Escitalopram falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on a liver enzyme called CYP3A4 to help break down escitalopram (Lexapro). Lab studies suggest that compounds in cannabis, especially CBD and CBG, can slow this enzyme down. In theory, that could let a bit more escitalopram linger in your system, which might nudge up its usual side effects like drowsiness, nausea, dry mouth, or feeling more sedated than normal.
I want to be honest with you though, this is based only on test-tube research, not studies in actual people, so we really don't know how big a deal it is day to day. It's worth a quick chat with your doctor or pharmacist, especially if you notice new grogginess or jitteriness. Please don't adjust your Lexapro on your own.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the gist for Lexapro (escitalopram). Your liver uses an enzyme called CYP2C19 to help clear this medication. The CBD in cannabis can slow that enzyme down, so escitalopram may hang around at higher levels than usual. We've actually seen this with its close cousin, citalopram, where CBD pushed blood levels up.
In plain terms, that could mean more of the usual SSRI side effects: drowsiness, nausea, headache, trouble sleeping, or feeling jittery. At higher doses, escitalopram can also affect your heart's rhythm (a QT prolongation concern), so that's worth keeping in mind. Keep in mind this is a theoretical interaction based on how the drug is processed, not proven in large studies. Still, before mixing the two, please check in with your pharmacist or doctor.
