Here's the gist for this pairing. Nifedipine is one of those medicines your body processes using an enzyme called CYP3A4, and that same enzyme also helps break down THC, the main active compound in cannabis. The concern on file is that nifedipine can act as a mild brake on CYP3A4, which could let THC linger a bit longer than usual. In plain terms, that might make the cannabis feel stronger or last longer, with more drowsiness, dizziness, or a faster heartbeat.

I'll be honest, this is a theoretical interaction based on how these things work, not something proven in real patients, so I wouldn't lose sleep over it. Nifedipine itself is not a prodrug, so its blood-pressure benefit isn't reduced here. Still, since both can drop your blood pressure and cause lightheadedness, please check with your doctor or pharmacist before combining them.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist for nifedipine. Your liver uses an enzyme called CYP3A4 to break this medication down and clear it out. Lab studies suggest that compounds in cannabis (especially CBD and CBG) can slow that enzyme down a bit. If nifedipine gets cleared more slowly, more of it can hang around in your blood, which could nudge its blood-pressure-lowering effect a little too far.

In real terms, that might mean things like dizziness, flushing, headache, swollen ankles, or a faster heartbeat. The honest catch is that this is based on test-tube research, not solid human studies, so it's more of a theoretical heads-up than a proven problem. Still, if you use cannabis and notice any of those symptoms, give your pharmacist or doctor a quick call before changing anything.

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
