Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hordenine interacts with.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist: your body relies on an enzyme called CYP2D6 to break down dextromethorphan and clear it out. Hordenine has been shown, in test-tube studies only, to weakly slow that same enzyme down. If that translated to real life, less of the cough medicine would get cleared, so a bit more could hang around than usual.

In practice, that might mean slightly more of dextromethorphan's typical effects, like drowsiness, dizziness, or feeling foggy. But I want to be honest with you: this is a theoretical, lab-only concern and the effect is rated minor. For most people taking an occasional cough product, it's unlikely to cause trouble. If you take hordenine regularly or feel unusually woozy, just check in with your pharmacist or doctor first.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
