Why this applies to Galantamine

Galantamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hordenine interacts with.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the situation with these two together. Your body partly relies on a liver enzyme called CYP2D6 to clear galantamine. In lab studies (test tube only, not people), hordenine appears to weakly block that enzyme. If that happened in the body, galantamine could clear a bit more slowly and build up slightly, which might nudge up its usual side effects like nausea, vomiting, diarrhea, dizziness, a slower heartbeat, or loss of appetite.

I want to be honest about the evidence though: this is theoretical and graded as minor. We simply don't have human data showing it's a real-world problem. Galantamine is not a prodrug, so the direction (mildly stronger effect) is what we'd watch for. Still, please run it by your pharmacist or doctor before combining them.
