Why this applies to Meperidine

Meperidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hordenine interacts with.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist with meperidine (Demerol) and hordenine. Your liver uses an enzyme called CYP2D6 to help clear meperidine. In a lab dish, hordenine seems to gently slow that same enzyme down. If that carried over into people, meperidine could clear a little more slowly and build up slightly, which in theory could mean more drowsiness, dizziness, slowed breathing, or constipation.

That said, I want to be honest about the evidence here. This is only test-tube data, the inhibition was described as weak, and we don't have real human studies showing a meaningful problem. So I wouldn't lose sleep over it, but since meperidine is a strong opioid, please run any hordenine supplement past your pharmacist or doctor first.
