Propafenone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hordenine interacts with.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the situation with propafenone and hordenine. Your body clears propafenone partly through a liver enzyme called CYP2D6. In lab dish studies, hordenine appears to mildly block that same enzyme. In theory, slowing that pathway could let propafenone build up a bit more than usual.

Why does that matter? Propafenone is a heart rhythm drug, so higher levels could mean more of its effects, things like dizziness, a slow heartbeat, fatigue, or in some cases a worsening rhythm problem. That said, this is based only on test-tube data and the effect looks weak, so it's far from certain it happens in real people. Still, since this is a heart medication where small changes can matter, I'd run it by your pharmacist or doctor before combining them.

About Propafenone

What Is Propafenone Used For?

Brand names: Rythmol, Arythmol

Propafenone is used to help control abnormal heart rhythms. It is prescribed to delay the return of paroxysmal atrial fibrillation or atrial flutter (episodic irregular heartbeats in the upper chambers) and paroxysmal supraventricular tachycardia (PSVT, sudden fast heartbeats starting above the heart's lower chambers) — but only in people who do not have structural heart disease.

It is also used to treat life-threatening ventricular arrhythmias (dangerous fast rhythms starting in the heart's lower chambers). Because of serious safety concerns, it is not recommended for minor or non-life-threatening heart rhythm problems.
