Modafinil falls under STIMULANT DRUGS — the group Hordenine interacts with.

Hordenine is structurally similar to N-methyltyramine and synephrine, constituents in bitter orange known to have stimulant properties. Theoretically, taking hordenine with drugs with stimulant properties might increase the risk of hypertension and other adverse cardiovascular effects.
Some of these drugs include amphetamine, caffeine, methylphenidate, pseudoephedrine, and many others.

Here's the simple version: both hordenine and modafinil have stimulant-type effects on your body. Modafinil is a wakefulness medication that can already bump up your heart rate and blood pressure a little. Hordenine, which is chemically related to the stimulants in bitter orange, may nudge your norepinephrine (a natural 'rev it up' chemical) in the same direction. Stacking the two together could, in theory, add up and raise the odds of higher blood pressure, a faster or pounding heartbeat, jitteriness, or trouble sleeping.

I want to be honest that this concern is theoretical, based on how these compounds work rather than real patient studies, and hordenine itself hasn't been well studied in people. Still, since modafinil already has cardiovascular effects, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you have any heart concerns.

What Is Modafinil Used For?

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
