Why this applies to Codeine Sulfate

Codeine Sulfate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hordenine interacts with.

Hordenine weakly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, hordenine might increase the levels of CYP2D6 substrates.
Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with codeine: it's actually a prodrug. On its own it does very little for pain. Your liver has to convert it into morphine using an enzyme called CYP2D6, and that morphine is what does the heavy lifting. So anything that slows down CYP2D6 tends to leave you with less pain relief from codeine, not more.

Hordenine has only been shown to weakly block CYP2D6 in lab dishes, not in real people, so this is mostly theoretical and rated minor. In practice you may not notice a thing, but if your codeine seems to stop working well, that could be a clue. Before combining them, it's worth a quick chat with your pharmacist or doctor.
