Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the basic idea with entrectinib (Rozlytrek): your liver uses an enzyme called CYP3A4 to break it down and clear it out. Lab studies suggest that some compounds in cannabis (CBD in particular) can slow that enzyme down. If that happens in real life, entrectinib could clear more slowly and build up in your body, which may mean stronger side effects like dizziness, tiredness, trouble with balance or thinking, constipation, or changes in heart rhythm.

I want to be honest about the strength of this, though. It's based on test-tube research, not on people, so it's more of a theoretical concern than a proven one. Still, because entrectinib is a serious cancer medicine, please don't start, stop, or change anything on your own. Run the cannabis question by your oncologist or pharmacist first.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
