Entrectinib falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with this combination. Your cancer medication, entrectinib (Rozlytrek), can already stretch out part of your heartbeat called the QT interval. American hellebore is a very toxic plant that also affects the heart, and it can lengthen that same interval on top of dangerously slowing your heart rate and dropping your blood pressure.

Stacking these two together raises the risk of a serious, sometimes life-threatening rhythm problem called torsades de pointes, which can cause fainting, a racing or fluttering heartbeat, or worse. The evidence here is limited, but American hellebore is poisonous even in small amounts, so I would steer well clear of it. Please don't change anything on your own. Talk with your oncologist or pharmacist first.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC) — a type of lung cancer where the ROS1 gene is rearranged — as confirmed by an FDA-approved test.

Rozlytrek is also approved for adult and pediatric patients older than 1 month with solid tumors that have an NTRK gene fusion (a specific genetic alteration), detected by an FDA-approved test, that have spread or cannot be removed without causing serious harm, and have worsened after prior treatment or have no good treatment alternatives. This NTRK indication was granted accelerated approval based on tumor response rates; continued approval may depend on results from confirmatory trials.
