Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the simple version: your body relies on a liver enzyme called CYP2D6 to break down dextromethorphan, the cough suppressant in products like Benylin and Theraflu. In test-tube studies, quercetin appears to slow that same enzyme down. If that happened in real life, dextromethorphan could linger in your system a bit longer, which might mean more of its usual effects, like drowsiness, dizziness, lightheadedness, or feeling a little foggy.

The important catch is that this has only been seen in lab studies, not in actual people, so it stays theoretical. Food amounts of quercetin are very unlikely to cause trouble. If you take high-dose quercetin supplements and use cough medicine regularly, just check with your pharmacist or doctor first.

Dextromethorphan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your cough medicine, dextromethorphan, is partly cleared by a liver enzyme called CYP3A4 (its bigger pathway is actually CYP2D6, but 3A4 helps too). Quercetin can mildly affect CYP3A4. If it slows that enzyme down, a bit more dextromethorphan could hang around, which might mean more drowsiness, dizziness, or that floaty, mildly out-of-it feeling some people get. Interestingly, one human study suggested quercetin can sometimes speed CYP3A4 up instead, which could make the cough relief a little weaker.

The honest truth is this is mostly theory and lab-based, so the real-world effect is probably small, especially with quercetin from foods. Still, if you take a high-dose supplement, mention it to your pharmacist or doctor first.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
