CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about phenytoin: your liver clears it mostly through an enzyme called CYP2C9. Quercetin, in concentrated supplement form, appears to slow that same enzyme down. So in theory, taking high-dose quercetin alongside phenytoin could let phenytoin build up in your blood, nudging levels higher than your doctor intended.
Phenytoin is what we call a narrow therapeutic index drug, meaning the gap between a helpful dose and a too-high one is small. If levels creep up, you might notice things like dizziness, unsteady walking, slurred speech, double vision, or drowsiness. Right now this is mostly theoretical and based on studies with other drugs, not phenytoin itself, so don't panic. Food amounts of quercetin are very unlikely to matter. Before starting a high-dose supplement, just check with your pharmacist or doctor first.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the gist for phenytoin (Dilantin) and quercetin. On paper, quercetin can block an enzyme called CYP2C8, and phenytoin gets cleared partly through that same pathway. If that enzyme slows down, phenytoin could build up a bit in your system. Since phenytoin is a narrow therapeutic index drug, even small changes in its level can matter, possibly nudging you toward side effects like dizziness, drowsiness, unsteady walking, slurred speech, or double vision.
That said, this is mostly a theoretical concern from lab studies, not something proven in people, and a human study with a similar drug showed no real effect. Food amounts of quercetin almost certainly won't matter. Before starting a high-dose supplement, just run it by your pharmacist or doctor first.

What Is Phenytoin Used For?

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.
Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
