CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Budesonide
Budesonide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: budesonide leans heavily on a liver enzyme called CYP3A4 to be cleared from your body. Quercetin can block that enzyme in lab studies, and if it does the same in people, your budesonide could hang around longer and build up. With more steroid in your system, you might notice the usual corticosteroid effects creeping in, things like fluid retention, higher blood sugar, mood or sleep changes, or easier bruising over time.
That said, the evidence here is mostly from test tube and animal work, plus one mixed human study, so this is more theoretical than proven. Everyday amounts of quercetin from foods like onions and apples are unlikely to matter. Before starting a concentrated quercetin supplement, just check with your pharmacist or doctor.

What Is Budesonide Used For?

Budesonide inhalation suspension is used as a maintenance (long-term control) treatment for asthma in children 12 months to 8 years old. It also works as a preventive (prophylactic) therapy to reduce the frequency of asthma symptoms.
Important: This medication is not a rescue inhaler. It will not stop a sudden asthma attack. A separate fast-acting bronchodilator is needed for sudden symptoms.
