Why this applies to Formoterol

Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the gist of it: your body uses a liver enzyme called CYP2D6 to help clear formoterol. In test-tube studies, quercetin slows that enzyme down. If that same thing happened in people, formoterol could build up a bit and lead to more of its usual side effects, things like a fast or pounding heartbeat, shakiness or tremor in the hands, headache, or feeling jittery.

The important part: this has only been seen in lab dishes, never actually reported in real patients, so it's more theory than proven concern. Everyday amounts of quercetin from foods are very unlikely to matter. If you take high-dose quercetin supplements, just mention it to your pharmacist or doctor so they can keep an eye on things. Don't change your inhaler on your own.

Formoterol also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: high-dose quercetin can slow down a liver enzyme called CYP2C9, which clears certain medications from your body. When that enzyme is blocked, those drugs can hang around longer and build up. For Formoterol, that could in theory mean a bit more of the medicine in your system, which might show up as the usual long-acting beta agonist effects feeling stronger, things like a faster or pounding heartbeat, shakiness or tremor, jitteriness, or a headache.

That said, this is mostly theoretical for Formoterol. The real human data involved different drugs (like diclofenac), and Formoterol is only partly handled by this pathway. Food amounts of quercetin are very unlikely to matter. Before starting a concentrated quercetin supplement, just run it by your pharmacist or doctor.

About Formoterol Oral Inhalation

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
