Why Are There 2 Interactions?
Quercetin interacts with whole categories of medication. Colchicine falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Colchicine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the thing with colchicine: your body relies on an enzyme called CYP3A4 to help break it down and clear it. Quercetin, especially in concentrated supplement doses, may slow that enzyme down a bit. If colchicine clears more slowly, levels can creep up, and that matters because colchicine has a fairly narrow safety window. Higher levels can mean more nausea, diarrhea, stomach cramping, and in rarer cases muscle weakness.
I want to be honest about how strong this evidence is, though. It's mostly lab and animal data, and quercetin has actually shown mixed effects on this enzyme. Food amounts of quercetin (apples, onions, berries) are very unlikely to be a problem. Before starting a high-dose supplement, just run it by your pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES
2 of 2Colchicine also falls under P-GLYCOPROTEIN SUBSTRATES.
There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the thing with colchicine and quercetin. Your body relies on a pump called P-glycoprotein (and a liver enzyme called CYP3A4) to escort colchicine out and keep its levels in check. Lab and small human studies suggest quercetin can block that pump. If less colchicine is being pushed out, more can stay in your system, which means stronger effects and a higher chance of side effects like nausea, stomach cramps, and diarrhea. In larger buildups, colchicine can become genuinely toxic.
Colchicine isn't a prodrug, so this goes in the direction of more drug, not less. The evidence here is still on the preliminary side, and food amounts of quercetin are unlikely to cause trouble. Concentrated supplements are the real question. Please check with your pharmacist or doctor before combining them.

What Is Colchicine Used For?
Brand names: Colchicine, Colsalide Improved, Mitigare, Colcrys, Gloperba, Lodoco
Colchicine tablets and capsules are used to prevent gout flares in adults, and colchicine tablets are also used to treat acute gout flares when taken at the first sign of a flare. Colchicine tablets are additionally approved for familial Mediterranean fever (FMF) — a hereditary inflammatory condition — in adults and children 4 years and older.
Lodoco (colchicine tablet) is specifically indicated to reduce the risk of heart attack, stroke, coronary revascularization, and cardiovascular death in adults with established atherosclerotic disease or multiple cardiovascular risk factors. Colchicine is not a pain reliever and should not be used for pain from other causes.
