Morphine Sulfate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the honest picture on this one. The concern listed is that quercetin might block an enzyme called CYP2D6, which could let drugs that depend on it build up in your body. For morphine, though, this connection is pretty weak. Morphine isn't really cleared by CYP2D6 in any major way (it's mostly handled by a different process called glucuronidation), so I wouldn't expect quercetin to push your morphine levels up much.

On top of that, this flag comes only from lab dish studies, and it has not been seen in actual people. Eating quercetin-rich foods like apples or onions is very unlikely to matter. If you're considering high-dose supplements while on morphine, just run it by your pharmacist or doctor first and watch for extra drowsiness or slowed breathing to be safe.

P-GLYCOPROTEIN SUBSTRATES

Morphine Sulfate also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist for morphine: it's moved around your body partly by a transporter pump called P-glycoprotein. Quercetin, in concentrated supplement doses, may slow that pump down. In theory, that could let a little more morphine get absorbed and build up in your system, nudging up its effects.

With an opioid like morphine, more in your system can mean more drowsiness, dizziness, constipation, and at the far end, slowed breathing. That said, this is mostly theoretical and based on weak studies in other drugs, not morphine itself. Everyday food amounts of quercetin are very unlikely to matter. If you're considering a high-dose supplement while on morphine, please run it by your pharmacist or doctor first.

Morphine is used to manage acute (short-term) and chronic (long-term) pain that is severe enough to need an opioid and that hasn't been adequately treated by other options. The higher-concentration liquid form is reserved for adults who are already opioid-tolerant — meaning their body is used to opioid medications.

Because of real risks of addiction, misuse, and overdose, morphine is only prescribed when other pain treatments have not worked or are not expected to work. It is not intended for long-term use unless the pain continues to be severe and no alternatives are suitable.
