Why this applies to Fentanyl

Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the honest picture with quercetin and fentanyl. The concern on file is that quercetin may block an enzyme called CYP2D6, which could in theory let drugs cleared by that pathway build up. Fentanyl is a strong opioid, and if it lingered longer than expected, you could see more sedation, slowed breathing, confusion, or dizziness. With a powerful pain medicine like this, even small shifts deserve respect.

But here's the important catch: this comes only from lab (test tube) studies, and it has never actually been shown in real people. On top of that, fentanyl is mostly broken down by a different enzyme (CYP3A4), not CYP2D6, so this interaction is likely minor at most. Still, please run any quercetin supplement past your doctor or pharmacist first.

Fentanyl also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the connection: your body relies on a liver enzyme called CYP3A4 to break down fentanyl. Quercetin, in high supplement doses, may slow that enzyme down a bit. If it does, fentanyl could clear from your system more slowly and build up. With a strong opioid like fentanyl, that's worth taking seriously, since extra drug can mean deeper drowsiness, confusion, and dangerously slow or shallow breathing.

That said, the evidence here is thin. It comes mostly from lab and animal studies, and one human study actually hinted quercetin might do the opposite. The amount of quercetin in everyday foods is very unlikely to matter. Before starting a concentrated supplement, please run it by your pharmacist or doctor first.

About Fentanyl

What Is Fentanyl Used For?

Brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, Subsys

Fentanyl transdermal system is prescribed for severe, persistent pain that cannot be controlled with other pain medicines, including shorter-acting opioids. It is only for people who are already opioid-tolerant — meaning they've been taking a significant daily opioid dose for at least a week.

It is not for short-term pain, post-surgical pain, mild pain, or use on an as-needed basis. Because of serious risks including addiction, overdose, and death, it is reserved for situations where other treatment options have failed or are not appropriate.
