Ibuprofen falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.
In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining quercetin with ibuprofen. Your kidneys use a little 'shuttle' called OAT3 to help clear ibuprofen out of your body. In lab studies, quercetin blocks that shuttle. In theory, slowing that exit could let a bit more ibuprofen linger, which might nudge up its usual side effects like stomach upset, heartburn, or, with regular use, effects on blood pressure or the kidneys.
That said, I want to keep this in perspective. This has only been seen in test-tube research, never confirmed in real people, so it's more of a 'good to know' than a red alert. Food amounts of quercetin are very unlikely to matter. If you take high-dose supplements or use ibuprofen often, just check in with your pharmacist or doctor first.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your kidneys use a little transporter called OAT1 to help clear ibuprofen out of your body. In lab dish studies, quercetin blocks that transporter. In theory, slowing that exit route could let a bit more ibuprofen hang around, which might nudge up its usual effects and side effects, things like stomach upset, heartburn, fluid retention, raised blood pressure, or stress on the kidneys.
I want to be honest with you, though: this is only seen in test-tube research, never reported in actual people. Everyday food amounts of quercetin almost certainly won't matter. If you're taking concentrated high-dose supplements alongside regular ibuprofen, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: your body uses a liver enzyme called CYP2C9 to clear ibuprofen out of your system. Quercetin, especially in concentrated supplement form, can slow that enzyme down. When the cleanup slows, a bit more ibuprofen can hang around in your blood, which may nudge up its usual side effects like stomach upset, heartburn, a higher chance of stomach irritation or bleeding, or strain on the kidneys.
Honestly, this is mostly theoretical and based on related drugs and lab work, not on ibuprofen directly, so the real-world effect may be small. Ibuprofen is a normal active drug (not a prodrug), so the direction here is simply more drug, not less. The quercetin you get from foods almost certainly won't matter. If you take high-dose quercetin regularly, just run it by your pharmacist or doctor first.

Brand names: Motrin, Children's Motrin, Advil, Children's Advil, Fenbid Forte Gel, Ibugel Forte, Arthrofen, Ebufac, Rimafen, Calprofen, Fenpaed, Galprofen, Nurofen for Children, Orbifen for Children, Brufen, Brufen Retard, Fenbid, Tab-Profen, Ibuprofen, Calodor
Oral ibuprofen products (tablets, coated tablets, and liquid-filled capsules) are used to temporarily relieve minor aches and pains from headache, backache, toothache, muscular aches, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever. The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).
Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
