CNS DEPRESSANTS

Morphine Sulfate falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind: morphine already slows down your brain and nervous system. That's how it eases pain, but it's also why it can make you sleepy and slow your breathing. Kava is another calming agent, working on the brain in a similar quieting way. Put the two together and those effects stack up.
So with morphine plus kava, you could end up more drowsy, foggy, dizzy, or unsteady than expected, and in a worst case your breathing could slow too much. Major safety groups actually advise against mixing kava with sedating medicines. One more note: both morphine and kava can stress the liver, so that's another reason to be careful. P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with kava and morphine. Morphine is moved around your body partly by a transporter called P-glycoprotein. In lab dishes, kava seems to slow that transporter down, which could in theory let a little more morphine build up than expected. With an active painkiller like morphine, that might mean stronger sedation, more drowsiness, slowed breathing, constipation, or feeling foggy.
But honestly, the human evidence here is thin. A study using a real-world kava dose didn't change levels of digoxin, another drug that relies on this same transporter, so any effect on morphine may be small. The bigger practical concern is that both kava and morphine can make you sleepy and calm, so stacking them can leave you more sedated. CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture for kava and morphine. On paper, kava might slow down an enzyme called CYP2D6, and morphine touches that pathway a little. But the actual human studies tell a reassuring story: when researchers gave people kava, it didn't meaningfully change how their bodies handled CYP2D6 medications. The lab worry just didn't hold up in real people, which is why this is rated minor and unlikely.
The bigger thing I'd flag isn't the enzyme at all. Both kava and morphine can make you sleepy and relaxed, so stacking them could leave you more drowsy, foggy, or slowed in your breathing. Kava can also stress the liver. Please loop in your pharmacist or doctor before combining these, especially with extended-release morphine.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

What Is Morphine Used For?

Morphine is used for pain that is severe enough to need an opioid and that other treatments cannot control well. It is meant for people who have not done well on, or cannot tolerate, other options.
Some uses depend on the product. Morphine sulfate tablets treat acute pain in adults and in children weighing at least 50 kg, and chronic pain in adults. Morphine oral solution (2 mg/mL and 4 mg/mL) treats acute and chronic pain in adults and acute pain in children 2 years and older. The 20 mg/mL oral solution is only for opioid-tolerant adults. Extended-release capsules and tablets, including MS Contin, are for severe, persistent pain that needs daily, long-term opioid treatment. Duramorph and preservative-free morphine injection can be given by vein, or into the epidural or intrathecal space (around the spinal cord). Infumorph and Mitigo are only for continuous epidural or intrathecal infusion for intractable chronic pain.
