Formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the gist with kava and your formoterol inhaler. Lab tests show that kava can slow down a liver enzyme called CYP2C19. The thinking is that if a drug leans on that enzyme to be cleared out, kava could let it build up a bit. The honest catch? This has only been seen in test-tube studies, never confirmed in real people, so it stays in the theoretical column.

On top of that, formoterol is mostly handled by different pathways, so this particular interaction likely isn't a big deal for you. If levels did creep up, you might notice more of formoterol's usual effects like a fast or pounding heartbeat, jitteriness, or a slight tremor. Just loop in your pharmacist or doctor before adding kava, partly because kava can be hard on the liver.

Here's the gist with kava and Formoterol. In test-tube studies, kava has been shown to slow down a liver enzyme called CYP2C9. If that same slowdown happened in people, drugs cleared by that enzyme could build up a bit and hit a little harder. Formoterol is an active medicine (not a prodrug), so in theory more of it sticking around could mean more typical side effects like a fast or pounding heartbeat, shaky hands, jitteriness, headache, or muscle cramps.

The important part: this is only seen in lab research so far, never confirmed in actual patients, so it's quite theoretical. Formoterol also isn't mainly handled by CYP2C9, which makes a real problem even less likely. Still, kava carries its own liver risks, so please run this combo by your pharmacist or doctor before using it.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with your formoterol inhaler (Foradil, Perforomist, and similar): the worry on paper is that kava might block a liver enzyme called CYP2D6, which helps break down formoterol. In a test tube, kava did slow this enzyme down. If that happened in your body, a bit more formoterol could linger, which in theory might mean more of its usual side effects like a fast or pounding heartbeat, shakiness, or feeling jittery.

The reassuring part: actual studies in people show kava does not meaningfully change how CYP2D6 drugs are handled. So this is a minor, unlikely concern. I'd still mention it to your pharmacist or doctor, mostly because kava can rarely stress the liver.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution (nebulizer; sold as Perforomist and as generic formoterol fumarate) is used for long-term, twice-daily maintenance treatment of bronchoconstriction (tightening of the airways) in people with chronic obstructive pulmonary disease (COPD). This includes chronic bronchitis and emphysema.

It is not for sudden COPD flare-ups and not for asthma.
