CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Chlorpheniramine
Chlorpheniramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture with kava and chlorpheniramine. On paper, chlorpheniramine is partly handled by a liver enzyme called CYP3A4, and some early lab tests hinted that kava might slow that enzyme down. If that were true, chlorpheniramine could linger and feel stronger, meaning more drowsiness, dry mouth, or grogginess. But the better human research doesn't back that up. In a study where healthy people took kava daily for a month, it had no real effect on CYP3A4.
So this particular enzyme interaction is rated minor and unlikely. My bigger concern is overlap of effects: both kava and chlorpheniramine can make you sleepy, so together they may leave you extra drowsy. Please check with your pharmacist before combining them.
