Hydrocodone Bitartrate falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both kava and hydrocodone calm down your central nervous system, so taking them together stacks those effects on top of each other. Hydrocodone is already a strong opioid that can cause drowsiness, slowed breathing, dizziness, and trouble thinking clearly. Add kava's own relaxing, sedating action and you can feel much more sluggish than expected, with a real risk of dangerously slow or shallow breathing.
This isn't a guess. Major guideline groups specifically warn against mixing kava with calming medications, and the evidence here is strong. There's also kava's separate concern for liver harm to keep in mind. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first so they can guide you safely.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the situation with kava and your hydrocodone. Hydrocodone is partly turned into its stronger pain-relieving form by a liver enzyme called CYP2D6, so it behaves a bit like a prodrug. In test-tube studies, kava seemed to block that enzyme, which in theory could mean less of the active painkiller forms and slightly weaker pain relief. But here's the reassuring part: actual studies in people show kava does not meaningfully change how the body handles CYP2D6 medications. So this is mostly a lab-only concern.
My bigger real-world worry is overlap: both kava and hydrocodone can make you drowsy, foggy, or slowed down, and kava is linked to rare liver injury. Please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the honest picture with kava and hydrocodone. On paper, kava was once thought to block CYP3A4, one of the liver enzymes that helps clear hydrocodone. If that were true, hydrocodone could build up and you might feel more of its effects, like extra drowsiness, slowed breathing, or constipation. But the better human research doesn't back that up. In a study where healthy people took kava daily for a month, it had no real effect on CYP3A4, so a meaningful enzyme interaction is unlikely.
My bigger concern is the overlap in how they make you feel. Both can relax you and make you sleepy, so combined they may leave you more sedated. Please check with your pharmacist or doctor before pairing them.
