Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern when you pair kava with doxycycline: both can be tough on the liver. Doxycycline, like many antibiotics, occasionally causes liver irritation, and kava has been linked to rare but real cases of liver damage, mostly with heavy or long-term use. Stacking two things that stress the same organ may add up.

To be fair, this is mostly theoretical and based on scattered case reports, not strong studies, so I don't want to alarm you. Still, it's worth watching for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Doxycycline is usually a short course, which lowers the worry. Please check with your pharmacist or doctor before combining them.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist with kava and doxycycline: doxycycline is partly moved around your body by a transporter called P-glycoprotein (P-gp). In test-tube studies, kava seemed to block that transporter, which in theory could let a bit more doxycycline build up. If that happened, you might notice more of doxycycline's usual side effects, like an upset stomach, nausea, or being a little more sensitive to sunburn.

The reassuring part is that this is mostly lab and animal data. A real study in people, using a different P-gp drug, showed no meaningful change, so any effect with doxycycline is uncertain at best. One bigger concern worth mentioning: both kava and, rarely, doxycycline can stress the liver, so it's smart to run this combo by your pharmacist or doctor first.

Doxycycline is an antibiotic. It treats infections caused by bacteria, and it does not work on viruses.

Several products carry a broad list of uses, including rickettsial infections, sexually transmitted infections, respiratory tract infections, eye infections, and anthrax after exposure. Some product labels also list malaria prevention, and use as an add-on treatment for acute intestinal amebiasis and severe acne. Doxycycline for Injection (Doxy 100, intravenous) is approved for infections caused by a listed set of microorganisms. Doxycycline capsules in the Oracea-type low-dose form are described in the evidence for inflammatory lesions of rosacea.
