Lofepramine falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Lofepramine is a tricyclic antidepressant, and like its drug family it already carries some sedating, calming effects of its own. Kava works on the brain in a similar quieting direction (researchers think it boosts GABA, the same calming signal many sleep and anxiety medicines tap into). Put the two together and those effects can stack up.

What that usually looks like in real life is feeling more drowsy than expected, sluggish, foggy, or unsteady on your feet. The bigger worry is that kava has been tied to rare liver injury, and lofepramine is processed by the liver too, so doubling up isn't ideal. This is a well-supported caution, not a wild guess. Please don't change either one on your own. Run it by your pharmacist or doctor first.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the gist with kava and your antidepressant Lofepramine. Lab studies (done in test tubes, not in people yet) suggest kava can slow down a liver enzyme called CYP2C19, which is one of the pathways that helps clear this medicine from your body. If that slowdown happened in real life, more Lofepramine could linger in your system, which might mean stronger effects like extra drowsiness, dry mouth, constipation, blurred vision, dizziness, or a faster heartbeat.

I want to be straight with you though: this is a theoretical concern based only on early lab work, not confirmed in humans, so don't panic. There's a bigger practical issue too. Both kava and Lofepramine can stress the liver, so combining them isn't something I'd take lightly. Please check with your pharmacist or doctor before mixing them, and never stop your antidepressant on your own.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture with kava and lofepramine (a tricyclic antidepressant). Lofepramine is partly handled by a liver enzyme called CYP1A2, and there was some early lab work and a single case report hinting that kava might slow this enzyme down. If that were true, more lofepramine could linger in your body, which can mean stronger side effects like drowsiness, dry mouth, blurred vision, constipation, or a faster heartbeat.

The good news: a better-quality human study found kava had no real effect on CYP1A2, so this concern looks unlikely in practice. I'd still mention it to your pharmacist or doctor, mainly because both kava and lofepramine can cause sedation, and kava carries its own rare liver risk. Don't change anything on your own.

Here's the short version on combining kava with lofepramine, your tricyclic antidepressant. In a test tube, kava looks like it might block CYP2D6, one of the liver enzymes that helps clear lofepramine. If that blocking happened in people, the drug could build up and you'd notice more of its usual effects, things like drowsiness, dry mouth, constipation, blurred vision, or a faster heartbeat. But here's the reassuring part: actual human studies show kava does not meaningfully change how the body handles CYP2D6 medications. So this is a minor, unlikely concern based on weak lab data.

My bigger flag is overlap of effects. Both kava and lofepramine can be sedating, and kava carries a rare risk of liver injury. Please check with your pharmacist or doctor before pairing them.
