Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the short version: your body clears theophylline mainly through a liver enzyme called CYP1A2. The worry on paper is that kava might slow that enzyme down, which could let theophylline build up. Since theophylline has a narrow safety window, too much can cause jitteriness, a racing or pounding heart, nausea, and trouble sleeping.

That said, the real-world evidence here is reassuring. A careful study in healthy people taking kava daily for about a month showed no meaningful effect on this enzyme. Only a lab test and one case report hinted at a problem, so this interaction is considered minor and unlikely. Still, kava carries its own rare liver risk, so check with your pharmacist or doctor before combining the two.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
