Doxepin falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to keep in mind: doxepin and kava both have a calming, sedating effect on your brain. Doxepin is a tricyclic that's used for sleep and to ease depression or anxiety, and it already tends to make people drowsy. Kava works in a similar direction, quieting nerve activity. Put the two together and those effects can stack up.

What that can look like in real life is feeling extra groggy, dizzy, slow to react, or unsteady on your feet, which raises the risk of falls or trouble driving. Expert guidelines actually advise against combining kava with sedating medicines. One more note: both doxepin and kava can stress the liver, so that's worth watching too. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with doxepin (Silenor). Doxepin is partly broken down by a liver enzyme called CYP2D6. In test-tube studies, kava looked like it might block that enzyme, which in theory could let doxepin build up and add to drowsiness, dry mouth, dizziness, or a groggy next-morning feeling. But here's the reassuring part: when researchers actually tested this in people, kava did not meaningfully change how the body handles CYP2D6 drugs. So this is rated minor and unlikely.

The bigger reason to check with your pharmacist or doctor first is overlap. Both kava and doxepin calm the brain, so together they may make you extra sleepy. Kava has also been tied to rare liver problems, so it's worth a conversation before mixing them.
