Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the simple version: this isn't about kava changing how rosiglitazone (Avandia) controls your blood sugar. The concern is the liver. Rosiglitazone is processed by your liver and can, in rare cases, stress it. Kava has its own reputation for causing liver trouble, with over 100 reported cases, mostly tied to heavy or long-term use. Put two things that can tax the liver together, and the worry is they could add up.

To be fair, this is a theoretical concern based mostly on scattered case reports, not strong studies. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Here's the gist with Avandia (rosiglitazone) and kava. Your body clears rosiglitazone mainly through a liver enzyme called CYP2C9. In lab studies, kava appears to slow that enzyme down. If that happened in real life, rosiglitazone could clear more slowly and build up a bit, which might mean stronger effects and a higher chance of side effects like fluid retention, swelling in the legs, weight gain, or low blood sugar (especially if you also take other diabetes medicines).

That said, this is still theoretical. It has only been seen in test-tube research, not in people, so I don't want to alarm you. There's also a separate point worth knowing: both kava and rosiglitazone can stress the liver, so combining them isn't ideal. Please check with your pharmacist or doctor before pairing these two.

What Is Rosiglitazone Used For?

Brand name: Avandia

  • Rosiglitazone is used along with a diet and exercise program and sometimes with one or more other medications to treat type 2 diabetes (condition in which the body does not use insulin normally and therefore cannot control the amount of sugar in the blood). Rosiglitazone is in a class of medications called thiazolidinediones. It works by increasing the body's sensitivity to insulin, a natural substance that helps control blood sugar levels. Rosiglitazone is not used to treat type 1 diabetes (condition in which the body does not produce insulin and therefore cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
