Glyburide falls under HEPATOTOXIC DRUGS — the group Kava interacts with.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: this isn't about kava changing how well your glyburide controls your blood sugar. Instead, the concern is the liver. Kava has been tied to rare but real cases of liver injury, and glyburide is one of those medicines that can occasionally stress the liver too. Put them together and you may be adding two small liver risks on top of each other.
I'll be honest, this is mostly a theoretical, anecdotal worry, not something proven for this exact pair, so I don't want to alarm you. Watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. Please check with your pharmacist or doctor before combining them, especially with long-term kava use.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
2 of 2Glyburide also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection between these two. Your liver uses an enzyme called CYP2C9 to break down glyburide and clear it from your body. In lab studies, kava appears to slow that same enzyme down. If that happened in real life, glyburide could hang around longer and build up, making it work a little too well. Since glyburide already lowers blood sugar, that could mean low blood sugar (hypoglycemia), with signs like shakiness, sweating, dizziness, confusion, or feeling suddenly hungry.
I want to be honest about the evidence here. This is only seen in test-tube research so far, not in actual people, so it's more of a theoretical concern than a proven one. Still, it's worth a quick chat with your pharmacist or doctor before combining them, especially since kava can also stress the liver.
