Kava interacts with whole categories of medication. Hydrocortisone falls into 2 of those categories, so it can interact with Kava in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
P-GLYCOPROTEIN SUBSTRATES
1 of 2Why this applies to Hydrocortisone
Hydrocortisone falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for kava and hydrocortisone. Hydrocortisone is one of many medicines carried by a cellular pump called P-glycoprotein (P-gp). In a test-tube setting, kava appeared to slow this pump down, which in theory could let a bit more hydrocortisone hang around. If that happened, you might notice more of the usual steroid effects, like fluid retention, higher blood sugar, mood changes, or trouble sleeping.
That said, the real-world evidence is thin. The concern comes mostly from lab work, and a human study using another P-gp medicine (digoxin) showed no meaningful change. So for most people this is unlikely to cause problems. Kava's bigger issue is its rare link to liver harm, so please run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Hydrocortisone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses an enzyme called CYP3A4 to help break down hydrocortisone, and there was a thought that kava might slow that enzyme down. If it did, hydrocortisone could linger a bit longer and you might notice more steroid-type effects like fluid retention, higher blood sugar, mood changes, or trouble sleeping. But that's the theory, not the reality.
The better-quality human research is reassuring. When healthy volunteers took kava daily for 28 days, it didn't actually change CYP3A4 activity. So a meaningful interaction here is unlikely. My bigger concern with kava is its rare link to liver injury, so please loop in your pharmacist or doctor before combining them, especially if your liver isn't fully healthy.

Hydrocortisone comes in many products, and each one is approved for different uses. Which one you get depends on the condition being treated.
Cortef tablets are used for adrenal insufficiency, congenital adrenal hyperplasia, rheumatic disorders (such as rheumatoid arthritis and gouty arthritis) and collagen diseases such as lupus. Hydrocortisone sodium succinate and Solu-Cortef injections are used when oral therapy is not feasible, for severe allergic states, skin diseases, and adrenal insufficiency. Alkindi (oral granules) is used as replacement therapy in children with adrenocortical insufficiency. Cortenema, Hydrocortisone Rectal Suspension and Cortifoam are used for ulcerative colitis or ulcerative proctitis. Topical hydrocortisone butyrate and valerate products, including Evesse lotion, are used for itchy, inflamed skin conditions and atopic dermatitis.
