Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Here's the thing to know if you take Valium (diazepam) and are thinking about kava: both of these calm down activity in your brain. Diazepam is a benzodiazepine that quiets overactive nerve signals, and kava's natural compounds (kavalactones) seem to work in a similar relaxing way. Stack them together and those calming effects can add up.

In plain terms, that could mean stronger drowsiness, dizziness, slowed reflexes, poor coordination, and trouble thinking clearly or staying alert. This is well documented enough that expert groups specifically advise against pairing kava with benzodiazepines. There's also a separate concern that kava can rarely harm the liver. Please don't change anything on your own. Talk with your pharmacist or doctor before combining these.

In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the thing to know if you're thinking about pairing kava with Valium (diazepam). Your liver uses an enzyme called CYP2C19 to help break diazepam down. Lab studies suggest kava can slow that enzyme. If that happened in real life, diazepam could build up a bit higher than usual, which might mean more drowsiness, dizziness, foggy thinking, or unsteadiness.

I'll be honest though, this has only been seen in test-tube research, not in actual people, so it's more of a theoretical concern. The bigger worry is that both kava and diazepam are calming and sedating on their own, and both can stress the liver. That combo deserves a real conversation with your pharmacist or doctor before you mix them.

Here's the gist with Valium (diazepam) and kava. Lab studies show kava can slow down a liver enzyme called CYP2C9, which is one of the helpers your body uses to break down diazepam. If that enzyme works slower, diazepam can hang around longer and build up. In plain terms, that could mean more of its usual effects, like extra drowsiness, dizziness, feeling foggy, or being unsteady on your feet.

Now, two important things. This is only seen in a test tube so far, not in people, so it's more of a theoretical concern than a proven one. It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture with kava and Valium (diazepam). On paper, both can calm the brain, and there was once a worry that kava might slow a liver enzyme called CYP1A2 that helps process certain medicines. But the better-quality human research doesn't back that up. In a study where healthy people took kava daily for a month, it had no real effect on that enzyme. So a meaningful change in your diazepam levels from this pathway is unlikely.

My bigger concern is simpler: both kava and diazepam are sedating, so together they could leave you extra drowsy, foggy, or unsteady. Kava has also been tied to rare liver injury. Please loop in your pharmacist or doctor before combining them, especially if you have any liver issues.

Here's the thing about pairing kava with diazepam (Valium): the lab worry was that kava might block CYP3A4, an enzyme your liver uses to clear diazepam. If that happened, diazepam could build up and leave you extra drowsy, foggy, or unsteady on your feet. But when researchers actually tested kava in healthy people, it didn't meaningfully change CYP3A4 activity, so this particular enzyme concern looks pretty unlikely in real life.

What Is Diazepam Used For?

  • Diazepam is used to relieve anxiety and to control agitation caused by alcohol withdrawal. It is also used along with other medications to control muscle spasms and spasticity caused by certain neurological disorders such as cerebral palsy (condition that causes difficulty with movement and balance), paraplegia (inability to move parts of the body), athetosis (abnormal muscle contractions), and stiff-man syndrome (a rare disorder with muscle rigidity and stiffness). Diazepam is also used along with other medications to control seizures. Diazepam is in a class of medications called benzodiazepines. It works by calming abnormal overactivity in the brain.
