CNS DEPRESSANTS

Chlorpromazine falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know about pairing kava with chlorpromazine (Thorazine): both calm down the nervous system, so using them together can stack those effects. Chlorpromazine already tends to make people feel drowsy, slowed down, dizzy, or foggy, and it can lower blood pressure (leaving you lightheaded when you stand up). Adding kava, which also has its own sedating effect, can push all of that further.
In real terms, that could mean stronger sleepiness, slower reflexes, and a higher risk of stumbles or falls. There's another reason to be careful too: both kava and chlorpromazine can stress the liver, so combining them isn't ideal. Please don't change anything on your own. CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Chlorpromazine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist with kava and chlorpromazine (Thorazine). Chlorpromazine is partly cleared by a liver enzyme called CYP2D6. In a test tube, kava extract slows that enzyme down, which in theory could let chlorpromazine build up and bring on more of its usual effects, like heavy drowsiness, dizziness, low blood pressure, or a dry mouth. The reassuring part is that when this was actually studied in people, kava did not meaningfully change how the body handles CYP2D6 medicines, so this concern stays mostly theoretical.
The bigger thing I'd flag is that both kava and chlorpromazine can make you sleepy and can stress the liver. That overlap, not the enzyme issue, is worth a quick chat with your pharmacist or doctor before combining them.

Chlorpromazine is used to manage psychotic disorders and treat schizophrenia. It also controls the manic type of manic-depressive illness, nausea and vomiting, and intractable (stubborn) hiccups.
It is also used for restlessness and apprehension before surgery, acute intermittent porphyria, and as an add-on in tetanus. It treats severe behavioral problems in children 1 to 12 years old and short-term hyperactivity with conduct problems. The oral products (concentrate and tablets) also list the general “management of manifestations of psychotic disorders.” The injection lists the other uses above.
